Chief Operating Officer
Dr. Muhammad Ajmal Khan
Dr. Muhammad Ajmal Khan is the Chief Operating Officer of the Punjab Healthcare Commission. He holds an MBBS degree from Allama Iqbal Medical College and a Master’s Degree in Human Resource Management from the University Of Manchester, UK. He stood 5th in the Central Superior Services Examination in a countrywide competition and was assigned to the District Management Group. Since then he has served at various senior level positions and has acquired a substantial work experience in the public sector, extended over 23 years. Before joining the Commission he was working as Secretary (Special Initiatives) to the Chief Minister Punjab.

Mr. Muhammad Azeem
Mr. Muhammad Azeem holds a Bachelor’s degree from the University of the Punjab. Mr. Azeem is a valued member of the PHC team who has more than 15 years of experience in administration, management and secretarial fields. Before joining the PHC he was working as Office Assistant at District Office Faisalabad, Red Crescent Society.

Ms. Ambreen Shehzadi
Ms. Ambreen holds an MPhil in Epidemiology & Public Health from University of Veterinary & Animal Sciences along with a Master’s degree in Bio-Statistics from University of the Punjab. She has experience of handling and analyzing complex data and has worked in both private and public sector. Before joining the Commission, she was working as Data Manager-Consultant with TRF+.

Dr. Riaz Ahmad Chaudhry
Dr. Riaz Ahmad Chaudhary holds an MBBS degree with a diploma in Ophthalmology from Punjab University, in addition to which he has a post-graduate diploma in Primary Health Care Policy from Leeds University, UK. He has exhaustive work experience of more than 36 years in Hospital Administration/Management, Public Health Management and Project Management in the public sector. Before joining the PHC, he was working as the Project Director, Faisalabad Institute of Cardiology. He has also been the Medical Superintendent of Services Hospital.

Dr. Muhammad Saleem Khokhar
Dr. Muhammad Saleem Khokhar is a medical doctor with an MBBS from Allama Iqbal Medical College and a Master’s degree in Health Administration from University of the Punjab. He has distinguished himself through his professionalism and integrity in a professional career spanning 25 years. He followed his service in the armed forces of Pakistan with several senior administrative positions in the private sector. Before joining the PHC, he was working as Medical Superintendent, Saira Memorial Trust Hospital.

Dr. Shafqat Ijaz
Dr. Shafaqat Ijaz earned an M.B.B.S degree from Nishtar Medical College, Multan and also holds a Master’s degree in Public Administration with specialization in Health Administration from University of the Punjab. He is a public health expert with more than 12 years of professional experience in the health sector at district, provincial and national levels in Pakistan. Before joining the PHC he was serving as Chief Operations Director at Marie Stopes Society.

Dr. Shahid Mehmood
Dr. Shahid Mehmood holds a master’s degree in Medical Administration from Quaid e Azam University and MBBS from Rawalpindi Medical College. He has vast experience of planning, organizing, management, implementation and coordination of medical / public health services and have been actively involved in training of medical / nursing & paramedical staff, research and development activities. Before joining the Commission, Dr. Shahid was serving as Director Medical Service HQ 4 Corps Lahore.

Dr. Farooq Ahmad Adeel
Dr. Farooq Ahmad has a Master’s degree in Community Health and Nutrition along with an MBBS from Quaid-e- Azam Medical College. He has extensive experience of working in the public sector and with NGOs for more than 19 years. Dr. Farooq Ahmad Adeel is currently working as an Deputy Director (Inspection) at the PHC. Before joining the PHC, he was providing his services as a Health Officer at Plan Pakistan.

Dr. Muhammad Hanif Sethi
Dr. Muhammad Hanif has earned an M.B.B.S degree from University of the Punjab and a Master’s degree in Advanced Medical Administration from Quaid e Azam University. Dr. Hanif has more than 30 years of work experience and has garnered a reputation for assiduity and forthrightness. He has served in the Pakistan Army (Medical Corps) and the Pakistan Air force. More recently, he was a member of the faculty at Amina Anayat Medical College, Sheikhupura.

Dr. Rehan Saeed Ahmad is a medical graduate from Allama Iqbal Medical College and also holds a Master’s degree in Obstetrics & Gynecology from University of the Punjab. His academic qualification also includes a Master’s degree in Healthcare Administration from H.T.A.A Kuantan, Malaysia. He is a dedicated professional and has a cumulative work experience of more than 20 years and prior to joining the PHC, he was working as an Administrator at Ittefaq Hospital (Trust) Lahore.

Dr. Muhammad Saleem, TI (M) is a medical graduate from Army Medical College, Rawalpindi and has a Master’s degree in Advanced Medial Administration from Quaid-e-Azam University, Islamabad. He has held many senior managerial positions in a professional career spanning 28 years. In recognition of his meritorious services, the Pakistan Army conferred the prestigious Tamgha-i-Imtiaz (Military) on him. He has served as the Head of PAF Masroor and as Assistant Commandant, CMH Lahore Medical College, before joining the PHC.

He has an extensive experience of more than 25 years in public health and administration. Before joining the Commission, Dr. Nasir was working as District Coordinator IRMNCH program, Vehari.

Dr. Syed Zafar Hussain
Dr. Syed Zafar is an experienced medical administrator, with leadership and quality management skills. He has rendered services to the armed forces for more than 28 years and has held the senior rank of Brigadier. Dr. Zafar holds a master’s degree in Medical Administration from Quaid-e-Azam University and a MBBS degree from Allama Iqbal Medical College. Before joining the PHC, he was the Commandant of CMH Nowshera.

Dr. Mubashir Ali
Dr. Mubashir Ali is a medical graduate from Quaid-e-Azam Medical College, Bahawalpur and has a Master’s degree in Medical Administration from Quaid-e-Azam University, Islamabad. He has held many senior administrative positions in the 28 years of his professional career. Prior to joining the PHC, he was employed as Head of Department, Medical Education at the Armed Forces Postgraduate Medical Institute, Rawalpindi.

Dr. Bakht Yawar holds a bachelor’s degree in Dental Surgery from Nishtar Medical College, Multan, and is currently pursuing his master’s in Health Management. He has extensive experience in the public sector, spanning more than 15 years. Before joining the PHC, he was working as Deputy District Officer Health in the Health Department of the Government of Punjab.

Dr. Abbas Reza holds a master’s degree in Medical Administration from Quaid e Azam University and an MBBS from Kind Edward Medical College. He also has a master’s degree in Business Administration with specialization in Human Resources from Punjab University and a postgraduate certificate in Hospital Management, University of Leeds, UK. Dr. Abbas is a seasoned employee with more than 28 years of experience in various Armed Forces medical set-ups including command of multilevel hospitals, field medical units. Before joining the PHC, he was working as Hospital Administrator at Gilgit Medical Centre- Aga Khan Health Service, Pakistan.

Dr. Badar ud Din
Dr. Badar ud Din holds an MBBS degree from Quaid-e-Azam Medical College along with a Master’s degree in Medical Administration from Quaid-e-Azam University, Islamabad. He has over 25 years of extensive experience in different command and staff appointments as medical doctor in Pakistan Armed Forces. Dr. Badar ud Din has also worked with private sector hospitals as well as with UNESCO/UNAIDS. Before joining the Commission, he was working as Civilian Medical Officer at combined Military Hospital, Mangla and Military Hospital, Rawalpindi.

Mr. Malik Muhammad Javed has a diverse professional experience of more than 8 years with national and international development agencies. He holds a Master’s degree from the University of the Punjab. Before joining the PHC he was working with the Technical Resource Facility. His experience with implementation of welfare schemes in the health sector render him uniquely qualified for his position at the PHC.

Mr. Awais Gulshan is a seasoned professional, possessing more than 10 years of experience in primary healthcare, at both district and provincial levels, and different reproductive healthcare projects funded by various donor agencies. He has a master’s degree in Sociology from Arid Agriculture University, Rawalpindi, and a diploma in Public Health Administration. Prior to joining the PHC, Mr. Awais was working as Deputy Senior Manager Operations at Marie Stopes Society.

Ms. Mariam Munir holds a Master’s degree in Economics from University of the Punjab and a Bachelor’s degree in Commerce from University of the Punjab. She has six years of work experience and has been employed as a Licensing Officer at PHC. Before joining this organization, Ms. Mariam Munir was working as an Assistant at the Planning & Development Department, Government of the Punjab.

Ms. Atia Fatima Qureshi holds a Bachelor’s Degree in Chemistry from FCC University. Before joining the PHC she has worked at PCSIR, Chem Gold Pvt. Ltd., and Government Jinnah Degree College. More recently she was working at the Punjab Agriculture & Meat Company as an Administration Officer.

Ms. Afifa Qadeer holds a Bachelor’s degree in Commerce from University of the Punjab and is currently pursuing her MPhil from the University of Lahore. Ms. Qadeer first joined the PHC team as an intern but because of her work ethic and diligence, she was offered a permanent position in the Licensing and Accreditation Department.

Mr. Ali Raza has a Bachelor’s degree in Computer Sciences from Cresset College Lahore. His position at PHC is that of Document Controller in the Licensing & Accreditation Directorate. He has an overall work experience of more than six years. Before joining PHC, Mr. Ali Raza was a part of BEAD Architects & Engineers, Abu Dhabi, as Senior Document Controller.

Prof. Dr. Riaz Ahmad Tasneem
Prof. Dr. Riaz Ahmad Tasneem is currently serving as the Director (Complaints) at the PHC. Dr. Riaz Ahmed Tasneem holds an MBBS degree from King Edward Medical College, along with a MS in Urology from Punjab University and FICS from International College of Surgeons, USA. He has an experience of more than 32 years in Academics, Administration and Medicine. He has also published 36 research papers on different projects in National and International journals. Prior to joining the PHC in September 2012, he was working at SIMS as Professor of Urology.

Director (Clinical Governance & Organizational Standards)
Dr. Mushtaq Ahmed
Dr. Mushtaq Ahmed holds an MBBS degree and a Masters in Public Health from University of the Punjab. He has more than 34 years of experience in the health sector, both in public and private. His areas of expertise include, inter alia, the planning and implementation of health related projects and public health development. His key achievements include successful implementation of a number of foreign funded, public health related programs/ projects of the Government of the Punjab. He has headed the DFID/USAID funded Technical Assistance Management Agency (TAMA) in Punjab, which managed to successfully implement 30 schemes of Technical Assistance for the Health Department and the Population Welfare Department, under his supervision. As Additional Secretary (Technical) he was in charge of the Technical wing of the Health Department, before he accepted the position of Director CG&OS at the PHC. Since then he has spearheaded the development of standardization documents for all Categories of Healthcare Establishment and the PHC’s Capacity Building Program for training the Expert Surveyors and the staff from the HCEs.

Dr. Anees Ahmad Qureshi
Dr. Anees Qureshi did MBBS from KEMU/Punjab University and MSc Medical Administration from QAU Islamabad. He has served in the Pakistan Army, the Nigerian Army, and Pakistan Rangers, as well as on key management positions such as Deputy Director Medical Services Pakistan Navy, MS AFTH Faisalabad, Director HS, BUITMS, Quetta, MS Ittefaq Hospital Lahore and as Public Health Consultant for Contech International. He has over 34 years of experience encompassing both public and private health sectors at home and abroad Public Health, Field Medicine, and has accumulated a wealth of experience in Public Health and Field Medicine, particularly with regard to Institutional Planning, Development, Clinical Governance, Standardization, Quality Assurance, Accreditation and Capacity Building. Developed a number of Developmental, Standardization and Capacity Building Documents which have been implemented. He is amongst those who contributed in the development of framework documentation including systems, processes and person specifications etc., providing basics for taking off of PHC. As Additional Director (CSD) he is contributing in the development of Standardization, Surveying and Assessment Documents for all Categories of HCEs. He Co-Facilitated all the Surveyor Trainings and is assisting in all Capacity Building activities, a vital role in the implementation of PHC Mandate.

Dr. Majed Latif
Dr. Majed Latif is a Public Health Physician. He has MBBS degree along with MCPS, MPH, MBA General Management and MBA Marketing. He has diverse experience of more than 20 years of working in public and private sectors with national and international health development partners at district, provincial and national level across the country. Developing teaching/training curricula and standards/protocols for healthcare service delivery, managing training projects and research, ensuring community participation for implementation of evidence based sustainable interventions and use of information for decision making remained his key areas of interest. As a PHC core team member participated in development of Technical / Training material, CG&OS/PHC Annual Reports, TORs/JDs and arranged a number of consultative workshops and conducted innumerable capacity building workshops for training HCPs and Expert Surveyors etc.

Dr. Mukhtar Ahmed Awan
Dr. Mukhtar Ahmed Awan did MBBS from Quaid-e-Azam Medical College Bahawalpur and MPH from Punjab University Lahore. He has a rich experience of working for more than 22 years in both the private and public sector up to mid-level management. Before joining the PHC he was working as Deputy District Officer Health in the Health Department Government of the Punjab. He has also worked with International development partners like UNFPA and International organizations e.g Save the Children and International Rescue Committee (IRC) heading different Health Programs/Projects as Provincial & District Manager in Punjab, Sind & AJK. He is currently working as Deputy Director (Trainings) and has been involved in all capacity building workshops of HCE staff & Surveyors/inspectors/ assessors as Co-Facilitator, He has also been part of assessment teams Conducting Inspections of HCEs through the L&A directorate. He has contributed in development of MSDS of different categories of HCEs besides contributing in dissemination & sensitization workshops/activities in collaboration with other Directorates at PHC. His area of expertise is RH & MNCH.

Dr. Shahid Amin holds an M.B.B.S degree and a post graduate diploma in Health Administration. Dr. Shahid Amin has held various senior positions in the Health Department, in a career spanning 26 years. Before joining the PHC, he was working as Deputy Secretary (Medical Education) at Health Department, Government of the Punjab.

Dr. Shafiq-ur-Rehman
Dr. Shafiq-ur-Rehman is a medical graduate from Nishtar Medical College. He has over 14 years of experience of working with INGOs in the implementation of public health related interventions, notably the UNICEF’s CMAM and CHARM projects. His extensive experience has helped him develop a deep understanding of challenges in the implementation of healthcare reforms in less developed districts of Punjab. His last appointment before joining the PHC was at the Health Department, as an EPI Coordinator at the District Office Health, Dera Ghazi Khan.

Mr. Waseem Shahzad is a seasoned employee with over 10 years of experience in the field of administration and office management. He holds a Master’s degree and has worked at both public and private organizations including INGOs. Before joining the Commission he was working as Project Officer at Technical Resource Facility (TRF).

Mr. Jamil Akhtar Ghauri
Mr. Jamil Akhtar Ghauri is a seasoned employee of the Commission holding 28 years of experience in the field of homeopathy. He holds a bachelor’s degree in Homeopathic Medical System and a diploma in Homeopathic Medical System. Mr. Jamil has been a member of numerous associations and societies which include Tehsil TB association Burewala and Khas Log welfare society. He is currently simultaneously serving as Educational Secretary of Asian Homeopathic Medical League, Pakistan Chapter since 2013. Before joining the Commission he served as Director Homeopathy in the Health Department.

Mr. Rana Muhammad Hassan
Mr. Rana Muhammad Hassan is MBA with Majors in HRM from National College of Business Administration & Economics and MSc Physics from the Punjab University. He has over nine years’ experience of working with public and private sector organizations such as PRMP, PDH and UCE before joining the PHC. Mr. Hassan is one of the pioneer employees of PHC and has worked in Administration, Licensing & Accreditation and now working with Directorate of Clinical Governance & Organizational Standards as Research Off

Ms. Rashda Parveen holds a Master’s degree in Biostatics, with a major in Statistics, from the University of the Punjab. She is currently engaged with the Directorate of Clinical Governance and Organizational Standards where her duties involve data analysis and providing support for Research and Development related work. Before joining the PHC, she was working as a biostatistician in Institute of Nuclear Medicine & Oncology and has also worked with the Punjab Health Department under the National Internship Program.

Mr. Asif Habib has more than 17 years of broad based and progressively responsible multifunctional experience in Management and Human Resources. Before joining the PHC he was working at Haleeb Foods Limited as Director HR & COO.

Mr. Farooq is an internationally and locally certified HR professional with more than 9 years of proven experience in human resource management, performance management, change management, training and development, administration and software development. He holds a master’s degree in Business Administration from NUCES, a bachelor’s degree in Computer Sciences from PUCIT and a diploma in Labor Laws from the Punjab University Law College. Mufti Abdus Salam is a Doctor of Pharmacy and the winner of the Gold Medal for the Faculty of Pharmacy, University of the Punjab. Mufti Abdus Salam has more than 33 years of professional experience and his last appointment before joining the PHC was as Chief Drugs Controller, Government of the Punjab. He has held various other senior positions, in the Drugs Control Administration as well as in different national and multinational pharmaceutical organizations.

Mr. Muhammad Ishaq is a skilled and qualified individual having multiple degrees and certificates in Law, Political Science, International Relations, History and Shariah Law. He has served in various organizations and has vast experience of investigation, administration, law and management. Mr. Ishaq has also served in the army as Major in General Head Quarters, Judge Advocate General Department which deals with all judicial cases of Army as well as civil suspects. Because of his meticulous service in the army he was also awarded Tamgha-e-Imtiaz (Military). Before joining the PHC, he was serving as Legal Advisor at PASSCO.

Mr. Khurram Mushtaq is an astute lawyer who has been associated with the practice of law for the past 14 years. Prior to joining the PHC, he was working as a Senior Associate at Rehman & Rehman Law Chambers.

Mr. Aamer has been associated with a number of prestigious educational institutes, media and governmental bodies where he has worked in different capacities. He also holds different certifications in graphics, animations and web designing from NCA, TDCP, and AIMS. Mr. Muhammad Majid has over a decade of experience in graphic design and design analysis, creative art direction, illustrations, animations, print and publishing media, corporate web design including development of software applications, and user interface (UI). Before joining the PHC, he was working as a Creative Director at Xoft Systems.
